Admissions & Enrollment Procedures

    1. Report Card (F138)
    2. PSA Birth Certificate
    3. Assessment or Entrance Exam
    4. Certificate of Good Moral Character
    5. (2) 1x1 ID Picture
Admission and Enrollment Procedure

STEP 1

Submit available requirements to the Principal's Office and secure an Admission Notice.

STEP 2

Present the completed Admission Notice Form to the Cashier’s Office and secure the Exam Fee Receipt.

STEP 3

Present the Exam Fee Receipt and Admission Notice Form to the testing area and take the exam.

STEP 4

Wait for the result. Secure an Admission Notice Form from the Principal's Office.

STEP 5

Go to the Encoding Area and fill out the form with the requested information necessary to produce a secure and accurate Student Record.

STEP 6

Present the Admission Notice Form to the Assessment Area.

STEP 7

Proceed to the Cashier for payment.

STEP 8

Present the Official Receipt at the Administration Office to receive the list of required school materials, class schedule, and section validation.

STEP 9

Purchase necessary books at the Finance Office and provide the receipt to the Books Area.

STEP 10

Secure and complete the School Service and Vehicle Sticker/Fetcher’s ID application form from the security officer area.

STEP 11

Have your ID picture taken at the Finance Office.
